## Usage note

Review from the approved sources and intended reader outcome, not from the generated draft alone. A named human reviewer must have authority, time, and relevant expertise to reject the output.

## How to use this template

1. Link the exact AI draft, provenance, approved brief, source set, and review version.
2. Evaluate usefulness, evidence, omissions, claims, voice, rights, safety, and audience fit.
3. Record every material human addition, correction, rejection, and escalation with a reason.
4. Reconcile the final candidate with sources and required disclosure or attribution.
5. Approve, return, or reject a named version and preserve the accountable review record.
