## How to use this template

1. Identify the parts of the workflow where AI was actually used.
2. Describe that assistance in specific, plain language.
3. Record the human review and factual verification performed.
4. Remove privacy, security, or accuracy assurances that cannot be confirmed.
5. Place the disclosure where the intended reader can reasonably find it.
