# Meaning-Preservation Edit Log

Record high-impact clarity edits and confirm that changes to terms, conditions, numbers, certainty, chronology, or scope did not alter intended meaning.

## Usage note

Use this log for edits that can change a reader's interpretation, obligation, eligibility, risk, or technical understanding. Routine spelling and punctuation corrections usually do not need individual records unless they alter meaning. This template supports editorial review but does not replace qualified legal, clinical, financial, scientific, safety, or localization approval where those are required.

## How to use this template

1. Identify edits with semantic risk and capture the exact source wording plus context.
2. Define the meaning that must remain, including scope, certainty, conditions, numbers, and sequence.
3. Record the proposed wording, edit operation, evidence, and potential failure mode.
4. Obtain the appropriate subject, editorial, and localization checks in full page context.
5. Approve, revise, or reject the edit and propagate the decision to every repeated occurrence.
