## Usage note

Use this record with your organization's approved rights language and qualified legal, privacy, safeguarding, or labor advice where needed. It is an editorial control, not a universal license agreement. Verify that the person granting permission controls the content and can consent. Apply additional safeguards for minors, vulnerable people, employees, private locations, sensitive events, and material containing other identifiable people.

## How to use this template

1. Identify the creator, original asset, source location, other people or rights, and any safeguarding or specialist-review need.
2. Send a clear request describing organization, purpose, channels, promotion, edits, credit, compensation, territory, duration, and withdrawal terms.
3. Verify the responder's control through an approved low-data method and capture an affirmative decision without preselected consent.
4. Link permission evidence to the approved file, allowed uses, edit boundaries, disclosure, expiry, and a unique internal rights identifier.
5. Check scope before every publication or reuse, record live locations, and act on expiry, correction, dispute, or withdrawal.
