How to use this template

  1. Identify participant, original statement, direct experience, date, relationship, incentives, and approved contact.
  2. Verify wording, facts, attribution, images, and any editing against the source.
  3. Define permitted channels, contexts, territories, duration, translation, paid use, and derivative assets.
  4. Obtain the approved consent or rights record and qualified review.
  5. Track live uses, expiry, corrections, withdrawal handling, and complete removal when required.

Verify the source and relationship

Confirm who made the statement, when, in what setting, and whether they had direct experience with the subject. Record current and statement-date roles, organization, customer or participant status, and any payment, discount, gift, contest entry, employee relationship, or other incentive. Preserve the original interview, message, or recording under approved controls. Do not invent a polished composite person or turn feedback about one interaction into a broad product claim.

Define exactly what is approved

Show the participant the proposed wording, attribution, image, context, and intended edits. State whether use covers a webpage, advertisement, social post, sales deck, email, event, print, translation, paid media, or third-party distribution, plus geography and duration. Explain cropping, excerpting, formatting, and translation review. Consent to one case study does not automatically authorize every future campaign or an AI-generated likeness.

Maintain accuracy and withdrawal handling

Check facts inside the testimonial separately and label personal perception appropriately. Avoid implying typical outcomes from an exceptional result; add necessary context and material relationship disclosure. Record the approved contact and process for corrections, expiration, and withdrawal requests according to governing agreements and policy. Reconfirm when role, organization, product, result, context, wording, channel, or duration changes. Remove expired uses from derivative assets, not only the source webpage.

  • Original: The fictional coordinator says one reminder feature helped her team during a four-week pilot.
  • Approved wording: Keeps “during our pilot” and does not become “saves every team hours.”
  • Relationship: The fictional organization received pilot access at no charge, disclosed beside the quote.
  • Scope: Product page and one email for six months; no paid social, translation, logo, or synthetic voice.
  • Expiry: Marketing owner checks all derivative uses at the end date and follows the approved removal process.

Frequently asked questions

Can grammar be cleaned up without approval?

Minor edits can change tone or meaning. Show proposed published wording and follow the approved review process.

Does a public social post grant testimonial rights?

Do not assume so. Obtain the permission and qualified review required for reuse, context, attribution, and platform.

Can one consent cover future campaigns?

Only within the explicit approved scope and governing agreement. New context, channel, edit, duration, or relationship may require renewed approval.

What if a participant asks to withdraw?

Follow the agreement, policy, and qualified guidance promptly. Track the request across source and derivative assets and communicate status appropriately.
